TSMG is a nonprofit, foundation-model organization. Established in 1997 to recruit, employ and manage practices for physicians and physician extenders, TSMG currently operates medical clinics in Keokuk, Hamilton and Warsaw. TSMG is associated with Community Health Centers of Southeastern Iowa. TSMG is also an Industrial Medicine provider serving over 80 regional industries.

Advance appointments are preferred, however, in urgent situations, every effort will be made for you to see a provider that day. We also offer a walk-in clinic on Saturday mornings for already established TSMG patients. Please be prepared to provide your name, address, date of birth, phone number, provider name and a description of your medical problem.
Our office hours are:
- Monday - Friday 8:00 – 5:00
- Monday - Thursday 5:00 – 6:30 (400 N. 17th location only)
- Saturday 8:30 - 11:30 (400 N. 17th location only)
Please call the clinic if you cannot keep a scheduled appointment.
Also, please remember that a parent or another adult must accompany anyone under the age of 18 with written permission from the parent to authorize health treatment.
TSMG provides quality medical care to anyone, regardless of age, race, nationality, employment, health insurance, income, or where you live. Our mission is to provide quality, accessible care to all who need it.
TSMG provides these health care services:
- A full range of primary care for infants through geriatrics
- Physician hospital visits
- Interpreter services
- Referral to specialized services including ENT, Orthopaedic, Surgical, and Counseling
- Benefit counseling to determine financial eligibility for health care services including prescriptions, lab, X-ray and consultations
- Woman, Infant and Children (WIC) access services
- Preventive medicine
- Industrial medicine and injuries
- Executive, Pre-employment and DOT Physicals
- For patients who qualify, a number of financial assistance programs are available at KAME Pharmacy, conveniently located next door to TSMG
We believe quality health care should be affordable to all. You will never be denied services because of your inability to pay.
After claims are filed with available insurance, the amount each patient is asked to pay will be determined by using a sliding fee scale/discount, based on family size and income. You must apply for the sliding fee scale/discount. You must provide us with documentation of your income.
The sliding fee scale co-pay begins at $15 (minimum fee) per clinic visit. Your portion of any fee must be paid at the time of your visit.
We accept most all health insurance coverage, including Medicaid, Medicare and HAWK-I insurance plans. Please bring your insurance card to all appointments.
Medicaid: We will not bill you if we have filed a claim with your current Title 19, Medicaid, Medipass, or Medicaid-HMO card.
Medicare: We accept standard Medicare assignments. We will apply the sliding fee scale to the outstanding balance that is not covered by Medicare payments.
