Keokuk Health Systems Notice of Privacy PracticesKeokuk Health Systems and all affiliates (ie. Keokuk Area Hospital, TSMG/CHC-SEIA, KAME, KHF) respects the rights and dignity of all our patients and maintains strict guidelines with regard to protecting the confidentiality of health information. Patients expect that any discussion or consultation involving their case will be conducted discreetly, that their medical record will be read only by individuals directly involved in their treatment, expect that all communications and other records pertaining to their care, including the source of payment for treatment, are treated as confidential. To accomplish this, records are kept under strict control. Computer terminals are accessible only to those with a need to have the information in order to do their job. Departments within the hospital are locked at all times when not staffed. This notice will tell you about the ways in which we may use and disclose medical information about you.Your confidential healthcare information may be released to other healthcare professionals involved in your treatment for the purpose of providing you with quality healthcare. Your confidential healthcare information may be released to your insurance provider for the purpose of the Keokuk Health System providers receiving payment for providing you with needed healthcare services. Your confidential healthcare information may be released to public or law enforcement officials in the event of an investigation in which you are a victim of abuse, a crime or domestic violence. Your confidential healthcare information may be released to other healthcare providers in the event you need emergency care. Your confidential healthcare information may be released to public health organizations or federal organizations in the event of a communicable disease or to report a defective device or untoward event to a biological product (food or medication). Your confidential healthcare information will be used by doctors, nurses, admissions and billing office staff, health information department staff, and other personnel who are involved in taking care of you at Keokuk Health Systems and affiliated entitities. There are some services provided to the Keokuk Health System entities through contracts with business associates, examples include state surveyors, the Joint Commission on Accreditation of Healthcare Organizations and other oversight agencies which when involved with these services we would provide health information to enable them to perform the service we asked them to do. These business associates, however, are required to safeguard your protected health information in the same manner as provided by this notice. Your confidential healthcare information may not be released for any other purpose than that which is identified in this notice except after receiving written authorization from you. You may revoke your permission to release confidential healthcare information at any time. Appointment RemindersYou may be contacted by Keokuk Health System providers to remind you of any appointments, healthcare treatment options or other health services that may be of interest to you. FundraisingYou may be contacted by Keokuk Health System providers for the purposes of raising funds to support the system's operations. Right to Request RestrictionsYou may have the right to restrict the use of your confidential healthcare information. However, Keokuk Health System providers may choose to refuse your restriction if it is in conflict of providing you with quality healthcare or in the event of an emergency situation. Right to Request Confidential CommunicationsYou have the right to receive confidential communication about your health status and designate where you wish to be contacted, for example, at home, at work. Right to Inspect and CopyYou have the right to review and photocopy any/all portions of your healthcare information. You must submit your request to the appropriate Keokuk Health System entity. If you request a copy of the information, we may charge a fee for the costs of retrieving, copying and mailing, and any other supplies associated with your request. Right to AmendIf you feel that any of the medical information we have about you is incorrect or incomplete, you may ask us to amend the information. You have a right to request an amendment for as long as the information is kept by our facility. To request an amendment, your request must be made in writing and submitted to the appropriate Keokuk Health System entity. In addition, you must provide a reason that supports your request. We may deny your request for an amendment if it is not in writing or does not include a reason to support the request. In addition, we may deny your request if you ask us to amend information that:
If the Keokuk Health System entity denies your request, a letter will be sent to you explaining the reason for the denial. Right to an Accounting of DisclosuresYou have the right to request an "accounting of disclosures." This is a list of the disclosures we have made of your medical information. We are not required to account for routine disclosures, for example, disclosures between Keokuk Area Hospital staff regarding your care. To request this accounting of disclosures, you submit your request in writing, to the appropriate Keokuk Health System entity. Your request must state a time period which may not be longer that six years and may not include dates before April 14, 2003. This first accounting you request within a twelve-month period will not include a cost for providing the disclosure list. For additional accountings, we may charge you for the costs of providing the list. We will notify you of the cost involved and you may choose to withdraw or modify your request at that time before any costs are incurred. Right to a Copy of This NoticeYou have the right to a copy of this notice. You may ask us to give you a copy of this notice at any time. This copy can be in the form of an electronic transmission or on paper. Facility DirectoryKeokuk Area Hospital maintains a Facility Directory. Information included in the directory may be provided to clergy and persons who ask for you by name. Clergy may also be provided with your religious affiliation. The protected health information includes the patient name, his/her location in the facility and the patient's condition in general terms that does not communicate specific information. This information may be included in the directory without your written consent or authorization, provided you were informed of the intended use or disclosure in advance and had the opportunity to either agree or prohibit the use or disclosure. If you wish to "opt out" of the directory, you must sign the "Objection to Listing in Patient Directory" form to prevent placement of information in the public directory and its subsequent disclosure. Changes to the NoticeAll Keokuk Health System entities will abide by the terms of this notice. Keokuk Health Systems reserves the right to make changes to this notice and continue to maintain the confidentiality of all healthcare information. The updated notice will be posted immediately on the Keokuk Health System website and made available to future patients. ComplaintsYou have the right to complain to the appropriate Keokuk Health System entity if you believe your privacy has been violated. If you feel your privacy rights have been violated, please mail your complaint to: ATTN: Risk Manager All complaints will be investigated. No personal issue will be raised for filing a complaint with any Keokuk Health System entity.
